Passport Center
The Ridgewood Library is now an officially designated U.S. passport application acceptance facility.
Passports are processed by appointment during Passport Center hours:
Monday and Wednesday: 3:30 to 8 pm
Saturday: 9:30 am to 4 pm
We have photo services for passports.
Don’t get turned away! Read the information below thoroughly and check Travel.State.Gov for all details before you register.
You must apply in person and use the DS-11 form if:
- You are applying for your first U.S. passport, or
- You are under age 16, or
- Your previous U.S. passport was issued when you were under age 16, or
- Your previous U.S. passport was lost, stolen, or damaged, or
- You need to renew an adult passport and answered “no” to any of the statements on the DS-82 Renewal form.
You need to bring the following:
- DS-11 Application.
- Fill out the appropriate sections in advance for a faster appointment using the U.S. State Department’s form filler.
- Proof of citizenship.
- Visit Travel.State.Gov for examples of evidence of citizenship.
- Proof of identity.
- One 2″ by 2″ color passport photo.
- The library can provide a photo for a fee.
- Checks or money orders.
- You will need a check for each DS-11 form and an additional one for the execution fee which is paid to the library. The execution fee is set by the U.S. State Department and is currently $35.
You must renew your passport by mail using a DS-82 form if you answer ‘yes’ to all of the following statements:
- You have your passport to submit with your application, and
- Your passport is undamaged other than normal wear and tear, and
- Your passport was issued when you were age 16 or older, and
- Your passport was issued within the last 15 years, and
- Your passport was issued in your current name or you can document your name change.
If you answered “no” to any of these questions, you must apply in person and use a DS-11 form. Any applicant with a five year passport must renew in person. You can learn more about passport renewals from the U.S. State Department.

DS-64. Lost or Stolen Passport
Two payments are required for all passports: one made payable to the U.S. Department of State and one made payable to the Ridgewood Public Library. Please note that the U.S. Dept of State does not accept cash or credit cards.
Passport Application Fees
Each passport requires a separate check or money order with the name and date of birth of applicant in the memo line of the payment. Checks or money orders for application fees must be made payable to US Department of State.
Passport Book
- Adults (age 16 and older): $130
- Children (under 16): $100
- For expedited passports: add $60 per application
- These fees must be paid by check or money order payable to the U.S. Department of State. The U.S. Department of State does not accept cash or credit card.
Passport Card
- Adults (age 16 and older): $30
- Children (under 16): $15
- For expedited passport cards: add $60 per application
- These fees must be paid by check or money order payable to the U.S. Department of State. The U.S. Department of State does not accept cash or credit card.
Passport Execution (Processing) Fee
There is a $35 execution (processing) fee per passport. The execution fee may be paid by cash, credit card, check or money order payable to Ridgewood Public Library.
Passport Photograph Fee
The Passport Agent will take a photograph for a fee of $15 per application. The fee for photographs may be paid by cash, credit card, check or money order payable to Ridgewood Public Library.
In many cases, you can apply for a renewal by mail. You must renew by mail if you answer “yes” to all of the following:
- You have your passport in your possession to submit with your application;
- Your passport is undamaged other than normal wear and tear;
- Your passport was issued when you were age 16 or older;
- Your passport was issued within the last 15 years;
- Your passport was issued in your current name.
The Ridgewood Library can provide photograph services for $15 per application. An agent can look at your renewal form for accuracy but cannot process or mail it for you.
If you answered “no” to any of these questions, then you must renew in person, by appointment, with a DS-11 form.
Under 16
All children, regardless of age including newborns and infants, must have their own passport.
All passport applicants under 16 must apply in person using Form DS-11. Applicants under 16 must appear with both parents or guardians in most cases and both parents must sign Form DS-11 in front of the Acceptance Agent and must be able to show parental relationship with any of the following:
- US Birth Certificate
- Consular Report of Birth Abroad
- Foreign Birth Certificate
- Adoption Decree
- Divorce or Custody Decree
Some documents, like a US Birth Certificate, show both US citizenship and parental relationship.
You must submit one of the following US citizenship documents for your child. Your document must be an original or certified, physical copy. Photocopies and notarized copies are not acceptable. Be prepared to surrender these documents. After your passport is printed, the Department of State will send the new passport to you and return your supporting documents in a separate mailing:
- Certified US Birth Certificate or Previously issued US Passport (may be expired)
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Certificate of Citizenship
The parents or guardians must also present proof of identity (valid, in-state driver’s license, for example). Please visit travel.state.gov for all acceptable forms of identification.
Passports issued to those under age 16 are valid for five years.
Ages 16 and 17
Applicants who are 16 and 17 years old must fill out Form DS-11 and apply in person. Applicants 16 or 17 years old should bring one parent or legal guardian who must bring their own ID.
You must submit one of the following US citizenship documents. Your document must be an original or certified, physical copy. Photocopies and notarized copies are not acceptable. Be prepared to surrender these documents. After your passport is printed, the Department of State will send the new passport to you and return your supporting documents in a separate mailing:
- Certified US Birth Certificate

After your passport is printed, the Department of State will send the new passport to you and return your supporting documents in a separate mailing.
If your passport is lost or stolen, you should report it immediately to protect yourself from identity theft. You can report a lost or stolen passport three ways.

Expedited

*Consider the total time it will take to get a passport when you are booking travel. Processing times only include the time your application is at a passport agency or center.
- It may take up to two weeks for applications to arrive at a passport agency or center. It may take up to two weeks for you to receive a completed passport after it is printed.
